Personal Identification Information (Pets)
Lucky Paws Pet Care customers will need to provide us with relevant and accurate information about their pets. This information is necessary in order for Lucky Paws Pet Care to carry out our dog walking and pop in service. Customers will be asked for pet name, breed, DOB/age and gender. This is important in enabling us to identify and address their pet as well as supply a vet with appropriate information in case of an emergency. We also need to know if the pet has been microchipped, and if they’re insured and regularly treated for worms and fleas. We also need to know if the pets vaccinations and boosters are up-to-date, and details of any past or present medical conditions, allergies and injuries they may have. Lucky Paws Pet Care will also ask if the dog is comfortable travelling in a vehicle, if they’re allowed dog treats, and if they’ve been aggressive or bitten another dog. We will also ask the customer if we have permission to take photographs of the Pet that can be used on our Facebook page and website. Lucky Paws Pet Care also needs information on the name and phone number of the pets vet in case of emergencies, and whether or not the customer gives us permission to authorize veterinary treatment on your behalf (and to what value). All information collected by Lucky Paws Pet Care from customers is absolutely necessary for us to carry out our services.

Lucky Paws Pet Care may collect and use user’s personal information (which is only collected should they choose to provide it in the contact form or if they are a Lucky Paws Pet Care customer who has willingly provided us with information) for the following purposes:

Your keys will be stored either on Lucky Paws Pet Cares person or in a locked safe and no information directly relating to you will be stored with your keys.
